Once I receive your email address and phone number, I will email you with a link to your secure, personal client portal. When you click on the link, you will be directed to complete an intake questionnaire, demographic questionnaire, and payment information form (optional). You will also be asked to review and e-sign documents, which includes a disclosure form and practice details.
